A filled-in example

What a finished incident report looks like: facts not blame, the time of the event, how bad it could have been, and an action with an owner and a date.

Incident report: carton fell from a pallet during unloading
Date and timeWednesday 14 October 2026, 10:40
LocationGoods-in, loading bay 3
Type of eventInjury, minor
People involvedWarehouse operative (injured). Forklift driver. Witness: goods-in supervisor.
What happenedWhile a pallet was being unloaded from a trailer, a shrink-wrapped carton slid off the top layer and struck the operative’s left forearm. The top layer had shifted in transit and was leaning towards the trailer door.
OutcomeBruising to the left forearm. First aid given on site. The operative returned to work the same shift.
Immediate actionUnloading of that trailer stopped. Two people re-stacked the leaning layer by hand. The load was photographed before it was moved.
Potential severitySerious. A heavier carton falling from the same height could have broken a bone or struck the head.
EvidenceThree photographs of the trailer and the pallet. A written account from the supervisor.
Next actionRaise the leaning load with the haulier, and add a load stability check before every unload. Owner: goods-in supervisor. Due: 21 October 2026.

Which form? Use this one for anything that went wrong. If nobody was hurt but someone could have been, the near miss report form template asks the right questions. For the legal record of an injury at work, use the accident book template.

The rules it follows

  • Facts, not opinions. “The walkway was unlit” can be fixed; “he was careless” cannot. The builder flags judgement words.
  • The event time, not the writing time. Vague words such as “yesterday” are flagged too.
  • Potential severity is what separates a trivial event from a serious warning, so it is always asked for.
  • An action with an owner and a date, or the finding never gets fixed.

Frequently asked questions

What should an incident report include?

The date and time of the event, the exact location, the type of event, the people involved and witnesses, what happened step by step, the outcome, the immediate action taken, the potential severity, any evidence, and the next action with an owner.

Is anything I type saved?

Nothing is uploaded or sent. While you work, the text is kept in this browser tab so a refresh does not lose it; closing the tab or pressing Clear removes it. Photos stay on your device and are never stored. If you accept analytics cookies, we count when the tool is used and a PDF is made, never what you type.

Can I use it for near misses or accident book entries?

Yes. Switch tabs at the top of the builder. Each has its own fields.

Can I report anonymously?

Leave the reported-by field blank and the report shows it as anonymous. Anonymous reporting often encourages honesty.
